Referral Criteria
Following our announcement on Wednesday 11th December, that St Cuthbert’s Hospice may be forced to make redundancies to protect in-patient palliative and end-of-life care, we are pausing current referrals for Bereavement Support at this time.
You may find our resources page helpful at this time. If you have any questions please contact the Bereavement Support Team on 0191 386 1170 or bereavementsupportteam@stcuthbertshospice.com.

The Bereavement Support Team provide counselling support to In-Patient and Living Well Centre guests, their families and carers at each stage of their health journey. We also have a children’s counselling service called The Jigsaw Project.
The Bereavement Support Team provide bereavement counselling to adults living in County Durham.
This care also extends to people in our community with no existing connection to the Hospice, but experience of a life-limiting illness. We offer bereavement counselling to adults in County Durham:
- who have a life-limiting illness themselves
- who have someone they care about that has a life-limiting illness
- who have experienced bereavement through a life-limiting illness (with a minimum of 6 months since death occurred)
This service is both free and confidential. Referrals can be made by self referral, a family member, primary health care team, mental health care teams or social workers using the forms at the bottom of this page.
Individuals will be placed onto a waiting list and then invited to an initial assessment. Following this initial assessment, the need for counselling will be established and arranged. As part of our triage, we may signpost your referral on to specialist services.
For general enquiries about Bereavement Support please call 0191 386 1170 or contact the team via email bereavementsupportteam@stcuthbertshospice.com.
