I would like to be considered for voluntary work with St Cuthbert’s Hospice and, should I become a volunteer, I agree to abide by the rules and policies of St Cuthbert’s Hospice concerning the duties of volunteers. If accepted as a volunteer at St Cuthbert’s Hospice I agree to maintain the confidential information of the organisation and its service users (without any time limit) and preserve the good name of St Cuthbert’s Hospice and not divulge any information that could be sensitive, confidential or detrimental to the Hospice. I agree to undertake additional checks prior to commencing volunteering if the role involves driving. I declare the information given on this form to be true and complete to the best of my knowledge.
